Muscle strain, sleeping awkwardly, overuse, arthritis, rotator cuff trouble, nerve irritation, and posture problems are far more common than anything serious. But there is an important exception that should not be ignored: sometimes pain felt in the right shoulder or right shoulder blade is not coming from the shoulder at all. It can be referred pain from the liver, especially when the liver or diaphragm is being irritated. The National Cancer Institute lists pain near the right shoulder blade among possible liver cancer symptoms, and the NHS also lists pain in the upper right abdomen or right shoulder as a possible symptom of liver cancer.

This matters because referred pain does not behave like an ordinary shoulder injury. When the liver becomes enlarged or a tumor irritates the diaphragm, the brain can misread the nerve signals and “feel” the pain in the shoulder instead of the true source. That means a person may have ongoing right shoulder discomfort even though the shoulder joint itself seems relatively normal. The article notes this is especially concerning when the pain is not clearly triggered by arm movement, feels more like a deep dull ache than a sharp mechanical pain, and does not improve with routine rest or standard treatment.

Other symptoms are often what help separate ordinary shoulder pain from something more serious. Warning signs include loss of appetite, feeling full quickly, unintentional weight loss, unusual fatigue, nausea, abdominal fullness or swelling, jaundice, dark urine, pale stools, itching, or a sense of pressure under the right ribs. These symptoms do not automatically mean liver cancer, but when they occur together with unexplained right shoulder pain, they deserve attention. Both the NCI and NHS list many of these same symptoms as possible warning signs of liver cancer.

The main point is not to create fear over every shoulder ache. The main point is to avoid missing a pattern that does not fit the usual orthopedic story. If the pain lasts more than a few weeks, has no clear injury behind it, continues at rest, is worse at night, or comes with other concerning symptoms, it should be evaluated. People who already have liver disease, cirrhosis, hepatitis, or other liver risk factors should take unexplained right shoulder pain even more seriously.

Nutritional Page: Liver-Supportive and Anti-Inflammatory Basics

Nutrition does not diagnose or cure liver cancer, but it does matter greatly for liver health, inflammation control, metabolic strain, and recovery support. A supportive plan generally emphasizes:

Adequate protein from clean sources to help maintain tissue repair and strength

Cruciferous vegetables such as broccoli, cabbage, cauliflower, and Brussels sprouts

Leafy greens for minerals and antioxidant support

Colorful vegetables and berries for polyphenols and phytonutrients

Omega-3 fats from sardines, salmon, and other quality fish

Fiber to support digestion, blood sugar control, and healthier hormone and toxin handling

Lower alcohol exposure or complete avoidance when liver stress is suspected

Reduced ultra-processed foods and excess sugar, which can worsen metabolic and fatty-liver patterns

Nutrients often considered in broader liver-support programs include:

Choline

Magnesium

B-complex vitamins

Vitamin D

Omega-3 fats

Selenium

Milk thistle

N-acetyl cysteine

Alpha-lipoic acid

These should be used thoughtfully, especially in anyone with known liver disease, abnormal lab work, or unexplained symptoms. Persistent right shoulder pain with red-flag symptoms needs proper medical evaluation, not just supplements.

For professional-grade supplements, the Fullscript dispensary can be accessed here:

https://us.fullscript.com/welcome/jkeefe-dc

Anyone with persistent unexplained shoulder pain, jaundice, abdominal swelling, rapid weight loss, poor appetite, dark urine, pale stools, or unusual fatigue should be medically evaluated promptly.
